How to Upload Documents

Step 1: Print off Employment Verification Form

  • This needs to be filled out with the applicants information and signed by BOTH the the dentist and the applicant.
  • The document is set up so you can type the information in before printing off to sign which is the preferred method. (Electronic signatures are acceptable if you have the capability.  This often requires Adobe Acrobat or a newer version of Adobe Acrobat Reader.)
  • Our Employment Verification Form is the ONLY one we'll accept.  A letter from the employer does not qualify.

Step 2: Scan or take a picture of your documents and save as either a PDF or JPG file (we cannot guarantee the ability to open any other format and may need you to resend them.)

Documents Needed:

  • Coronal Polishing: 1) Employment Verification Form
  • Sealants: 1) Employment Verification Form AND 2) Dental Assisting Permit showing Coronal Polishing Certification
  • Nitrous: 1) Employment Verification Form AND 2) HEALTHCARE PROVIDER or PROFESSIONAL RESCUER CPR Card (front & back)
    • HEALTHCARE PROVIDER (American Heart Association) and PROFESSIONAL RESCUER (American Red Cross) are the ONLY two forms of CPR we can accept.
    • Applicants who attempt to enroll with anything but these two forms (Heart Saver, CPR/AED, Community CPR, etc.) will be removed from the course until they can provide a card for the proper form of CPR.  If applicant chooses a refund over obtaining the proper CPR, a $15 processing fee will be charged.
  • Local Anesthesia: 1) Proof of liability Insurance AND 2) HEALTHCARE PROVIDER or PROFESSIONAL RESCUER CPR Card (front & back).
    • Liability Insurance can be the employers as long as the applicant is covered under the policy.
    • HEALTHCARE PROVIDER (American Heart Association) and PROFESSIONAL RESCUER (American Red Cross) are the ONLY two forms of CPR we can accept.

Step 3: Make sure documents saved properly and are readable.

Step 4: Upload your documents

  • Click on "Choose File"
  • Find where you saved the corresponding file (Employment Verification form, CPR, etc)
  • Select corresponding file and click on "Open".  File should now be uploaded.

Important Notes:

  • The application will not submit if every required field is not completed.
  • If the front and back of your CPR card is on the same page, upload it twice.
  • Applications submitted without the proper documentation are considered incomplete and will be immediately removed from the course until the proper documentation is provided.
