Employment Verification Forms and Covid-19 Requirement Forms must be filled out with the applicant's information and signed by BOTH the dentist and the applicant.
The Employment Verification Form is set up so you can type the information in before printing it to sign, which is the preferred method. (Electronic signatures are acceptable if you have the capability. This often requires Adobe Acrobat or a newer version of Adobe Acrobat Reader.)
Our Employment Verification Form is the ONLY proof of employment accepted. A letter from the employer does not qualify.
Step 2: Scan or take a picture of your documents and save them as either a PDF or JPG file
Documents needed by course:
Coronal Polishing: 1) Employment Verification Form
Sealant Application: 1) Employment Verification Form 2) Dental Assisting Permit showing "YES" next to Coronal Polishing
Nitrous Oxide Application: 1) Employment Verification Form 2) Everyone must have a Healthcare Provider/Professional Rescuer CPR card or eCard from a provider that does IN-PERSON skills testing (No online skills testing allowed.)
HEALTHCARE PROVIDER (American Heart Association) and PROFESSIONAL RESCUER (American Red Cross) are the ONLY two forms of CPR proof we can accept.
Applicants who attempt to enroll with anything but these two forms (Heart Saver, CPR/AED, Community CPR, etc.) from anyone other than one of those two agencies will not be added to the course until they can provide a card for the proper form of CPR.
Local Anesthesia: 1) Proof of Professional Liability Insurance 2) Everyone must have a Healthcare Provider/Professional Rescuer CPR card or eCard from a provider that does IN-PERSON skills testing (No online skills testing allowed.)
Liability Insurance can be the employers as long as the applicant is covered under the policy.

Step 3: Make sure that documents are saved properly AND ARE LEGIBLE (not too small, too dark or too light, etc).
Step 4: Upload your documents
Click "BROWSE"
Click "Choose File" and find where you saved the corresponding file (Employment Verification form, CPR, etc). You can also drag and drop the file if you choose.
Select the corresponding file and click "Open". The file should now be uploaded.
Important Notes:
The application will not submit if every required field is not completed.
If the front and back of your CPR card are on the same page, you can skip the "CPR Card Back" upload.
Applications submitted without the proper documentation are considered incomplete and will not be added to a course until the proper documentation is provided.
