Sealant Application ($425)
A dental assistant holding this expanded duty permit may place pit and fissure sealants.

Please read the following information before proceeding to the application at the bottom of the page. (Failure to do so could potentially delay or void your enrollment.) 

  • Hold a current Dental Assisting Permit from the Oklahoma Board of Dentistry showing they are certified in Coronal Polishing. (A letter stating you passed the Coronal Polishing course DOES NOT qualify an assistant to enroll. Board permit is the ONLY proof accepted.)
  • Must be currently employed as a chairside dental assistant.
  • Participants are REQUIRED* to provide a patient and four extracted teeth for this course. Please refer to the Patient Requirements below for details.
  • Participants are REQUIRED* to provide their own slow speed handpiece, attachments, and curing light. ODF does not have equipment available to loan. Please DO NOT enroll more employees than you have available handpieces.
*Failure to bring these required items to the course will result in dismissal from the course and the participant will be required to enroll and pay for a later course.
Before Enrolling
  • ALL APPLICATIONS MUST BE SUBMITTED ONLINE. Hard copy applications will not be accepted.
  • The enrollment fee must be PAID IN FULL, and the application approved before the applicant will be given a spot in the course.
  • You will not be considered for enrollment with an incomplete application (Read carefully to avoid a delay in your enrollment).
  • Applications will be processed in the order they are received.
  • Once payment is received and the application is approved, a final confirmation with a receipt will be sent.
