If you fail the course, the Board of Dentistry requires that you retake the entire course. Your enrollment will need to be reset and you will need to pay to re-enroll. To have your enrollment reset, please email Stacey Bonham and she will contact SeminarWeb on your behalf.
The Board of Dentistry no longer uses these forms and you no longer have to provide proof of taking x-rays to receive this expanded function. INSTEAD, you pay this $10 fee to the ODF and the they submit this fee to the Board of Dentistry when they send them the pass report.
For Coronal Polishing, Sealants, or Nitrous, you pay the fee at the course and the ODF submits the payment to the Board of Dentistry when we send them your results.
To pay the processing fee for your updated permit, you can access the instructions in the “Post Exam Instructions” under the “Other Resources” tab when you are logged into the Radiation Safety course – OR – through the email the ODF will send you the week after (typically Monday) you complete the course. Be sure to check your junk/spam folder for this email.
If you still need assistance paying the $10 processing fee for your permit, email [email protected] and provide your name and the course you enrolled in, and your DA Permit Number and we will send you the link. Please be advised that it can take up to 6 weeks for your new permit to be mailed to you by the Board of Dentistry.
